*Graduate School provides templates but AOUs have the flexibility to tailor the format of these to their requirements.

Candidate statement

This is a confidential questionnaire only shared with the Chair of the review panel, the Director of HDR, and the Graduate School. It should not be uploaded to the UQRDM. The statement is designed to find out how you are finding the process. You may ask your Chair to consider periods of leave or other events that have impacted your research progress. You can find an example of the questionnaire in the systems training guide.

You can draw upon your progress reports to help you explain this. Please consult with your supervisor about intellectual property and the disclosure of information prior to completing the candidate statement. Select 'unsure' if you have any doubts about IP and raise them in the meeting with your Progress Review Panel.

Your Candidate statement will be in the Progress Review request online form.

An Individual Development Plan

Every candidate should identify their professional development needs and create a professional development plan. Refer to your school/institute guidelines for any specific templates your school or institute may wish you to use.

We have also created an Individual Development Plan (IDP) for you to use if one is not prescribed by your local area. It includes a needs analysis to help you identify areas for development.

Portfolio of Activity

Refer to your school/institute guidelines for expectations. Many schools and institutes will request you demonstrate this by presenting an Academic CV. Note that you can download a list of any CDF sessions you attend via your Student Hub to add to this.

Project document(s)

Refer to your school/institute guidelines for expectations. Your project documents should be accompanied by an iThenticate report.

Principal advisor documents

Principal advisor statement

This is not something candidates are required to submit. Your Principal advisor will be asked to provide a confidential report directly to the Chair of the panel. You can find an example of the Principal advisor statement in the systems training guide.

The Principal advisor statement will be in the Progress Review request online form.

Chair documents

The Chair's report

This is a final report provided to the HDR candidate outlining recommendations and feedback from the Progress Review process. You can find an example of the report in the systems training guide.

We recommend sending it within 5 working days of the meeting.

The Chair's report will be in the Progress Review request online form for the Chair to complete.
