7. Types of documentation

Practical examples of documentation

The type of documentation you will be required to submit will depend on your circumstances. Here are a few examples.

Medical

A medical practitioner must assess your condition and provide a written assessment of your condition. This should include:

  • the dates on which you were impacted by your medical condition
  • how your condition affected your ability to complete the course
  • other information relevant to your request.

The medical practitioner must be independent, they cannot be a close relative or associate

Examples of supporting evidence
  • A signed medical certificate or letter(s) from the treating medical practitioner (doctor or specialist).
  • An official document from a hospital, such as admission documents, and a treatment and discharge summary that includes the dates of admission and discharge.

 

Family/personal

Special circumstances may include:

  • traumatic incidents  
  • significant personal problems
  • severe medical circumstances
  • death of a family member or significant other
  • sudden change of living arrangements
  • sudden change of financial situation
Examples of supporting evidence
  • A letter from a counsellor, psychologist, or psychiatrist. The letter must confirm how and when your circumstances affected your ability to complete the course.  
  • If you are a carer, the medical certificate or letter from a medical practitioner must state:
    • the medical status of the patient
    • the patient’s relationship to you and include relevant dates.    
  • If you are impacted by the death of a family member or significant other:
    • Death certificate
    • Funeral notice  
  • In circumstances when the relationship between you and the deceased is not immediately clear (for example, you have different surnames), please also provide evidence of the relationship, such as:
    • A birth certificate
    • A marriage certificate
  • Eviction notice
    • Must show name of occupants
    • Address of property
    • Date of eviction.  
  • A police report or a statement from a relevant authority. The report or statement must confirm the occurrence and date(s) of the trauma, misadventure, crime etc.  
  • Official letter from a relevant government department or statutory body or agency to support your change in circumstances:
    • Request for emergency housing
    • Request for emergency financial assistance.

Employment related

Special circumstances may include:

  • Loss of primary source of income for you or your parent/guardian if you are a dependant
  • Change of primary location of employment
  • Deployment overseas
Examples of supporting evidence
  • Notice of termination from the employer, which includes the date you were first notified and when the termination is effective
  • Notice from employer to change of primary location of employment, including relevant dates.
  • Copy of bank statements, list of expenses, bankruptcy notice.

Course-related

Special circumstances may include:

  • Significant change to the way a course is delivered after the census date
  • Significant change to the type and/or weighting of assessment after the assessment lockdown date  
Examples of supporting evidence

This can generally be verified with the relevant School or Faculty, so you don’t have to provide documentation, but any information you can provide will assist with investigating the matter. This may include:

  • Any communications you may have had with the Course coordinator, School and/or Faculty.
  • Any Blackboard or Class announcements outlining changes to the course and when the changes were advised/made.
