Arcare pre-placement requirements

What is required if I have a placement at an Arcare facility?

All students placed in an Arcare facility, regardless of discipline, will need to complete the following forms and upload evidence of completion to InPlace before placement commencement:

1. Arcare student acknowledgement and undertaking (PDF, 84.4 KB)
2. COVID-19 vaccination
3. NDIS worker orientation training certificate
4. Statutory declaration
5. National Hand Hygiene Initiative (NHHI) modules
6. Disability Worker Screening Clearance (yellow card)
7. Patient handling training

You will not be able to commence your placement until all items are completed.

What do I need to do?

Step 1

Read through and complete the Arcare student acknowledgement and undertaking form (PDF, 84.4 KB). Upload your completed form to InPlace.

Step 2

COVID-19 vaccinations are not mandatory for students, however, for reporting purposes, under the "COVID-19 Vaccination" heading in InPlace, please select YES. In addition, in the comment section please write an option to confirm correct status. If you are vaccinated comment "Yes". If you are not vaccinated comment "No". If you prefer not to disclose comment "Prefer not to answer".

Step 3

Please log into the NDIS website and complete the Worker orientation module: Quality, safety and you. Download your certificate of completion and upload it to InPlace.

Step 4

All students must complete a statutory declaration regardless of whether you have ever lived overseas. Statutory declaration must not include your full home address. Please include suburb and state only. Please use the statutory declaration instructions to complete this document and upload it to InPlace.

Step 5
